It is important to bring Insurance card and Photo Identification for the check in process. If there is not enough space for medications and/or past surgery history, patients are more than welcome to attach forms pertaining to their medical history.

All patients of record are asked to complete a new health history form every two years. This process can greatly reduce the possibility that the patient will inadvertently neglect to advise the podiatrist and staff of recent changes to his/her health status. As well as insurance information.
